Part 5: Required Documentation

VPK applicants MUST provide the following documentation:

1. 2019-2020 VPK Certificate of eligibility (COE). COE’s are issued online by the Community Coordinated Care for Children (4C) Office located at

    2220 E. Irlo Bronson Memorial Highway Unit #7 Kissimmee, FL 34744 Phone: (321) 219-6300, Monday - Friday (8:00a.m. – 4:30p.m.)

2. Child’s ORIGINAL Birth Certificate

3. Immunization Record

4. Current Physical Examination

5. Two (2) forms of Residence Verification-Proof of Residency (2 forms of documentation are REQUIRED. One MUST BE Lease or Mortgage*)

Part 6: General Information

There is NO transportation provided for VPK. Families are responsible for providing transportation for the VPK student to and from school.

Completing an application for VPK DOES NOT guarantee assignment to the first selected site. Placement decisions will be based upon VPK criteria. After criteria consideration, all other applicants will be placed accordingly.

All applications must be submitted onlineOnce your application is received and processed you will be contacted to set up an appointment to finalize the enrollment packet.
NOTE: Incomplete documents will not be processed.
