  • Middle School Athletic Physicals completed in the fall of 2020 are valid until May 31, 2021. 

    Note: ImPact Concussion testing is required for all Athletes prior to trying out.

    The form below needs to be filled out prior to any participation in Middle School Athletics.

    ***All Athletes will need at least a 2.0 GPA and NO F's in order to be eligible to play.
