SPORTS PHYSICAL PACKETS

Please note that A-D must be completed (including the completion of the impact test online) to be cleared to tryout for sports or participate in intramurals.  It must be delivered to the athletics department.  The HRMS Flyer has details on what we offer.

FOR ALL HRMS SPORTS UPDATES: please join the HRMS SPORTS REMIND by texting @hrmssports to 81010.  PLEASE SUBMIT A-C to the Athletics Department along with the completion date of D.  All computer Impact/concussion testing will be taken by the athlete on their own time.  It's an easy process to complete at home.  Please write the completion date on the bottom of Form A as instructed.
